Registration Form

I can’t wait to have your child as a student! Please use the attached form to submit some information so I can become more familiar with your child and his/her piano experience.

TUITION:
Monthly tuition is $140 for weekly one-hour lessons and is due during the first week of each month.

Lessons with my assistant, Jade, are $125 per month for weekly one-hour lessons.

Please submit payments via Zelle or cash. The Zelle account is connected to denaspianostudio@gmail.com.

There is also an annual $70 registration fee per student.

I am an ESA Vendor for home-schooling families!

ATTENDANCE:
Regular attendance is expected.​​​​ Please give notice if you know you will miss a lesson.​ Make-ups can only happen if there is an open slot in my schedule.  We can work that out through personal conversation. There is no refund for missed lessons.

REGARDING PRACTICE:
Students are encouraged to make the piano part of their daily rhythm. On each student’s binder, there is a practice schedule according to their level and skill set. 

Access to a well-maintained instrument is essential. Pianos function optimally when serviced twice a year. (I have a wonderful piano technician if you need a recommendation) 

Keyboards should be full-size with weighted keys.​

LESSON CALENDAR:
Please look at the binder your student(s) bring home.  All the dates for the upcoming year are already scheduled.  Please put them on your calendar!

MY GOAL:
My goal of teaching is to instill a love of music.  I strive to create opportunities to learn all types of music so that the student wants to practice.  I envision my students becoming musicians with a smile on their face and confidence in themselves to accomplish all their dreams.
